What is ketamine?

HOW IS IT ADMINISTERED?

Ketamine is a legal, safe, and effective medicine used to treat depression, anxiety, and PTSD. Its antidepressant and mood-lifting effects work fast: they can begin within 1–2 hours of treatment and last up to two weeks. Ketamine blocks the brain's NMDA receptors and stimulates its AMPA receptors — a process thought to form new synaptic connections and strengthen the neural circuits that regulate stress and mood. It also enhances neuroplasticity, which supports lasting improvement.

Ketamine can be given several ways: IV infusion, intramuscular injection, nasal spray, or sublingual lozenge. At Lewisburg Psychology, we partner with Journey Clinical to use the sublingual lozenge only.

What does it feel like?

The effects last about 45 minutes, and most clients find them pleasant. You may feel "far from" your body, with shifts in perception that often feel expansive. Your motor and verbal abilities are reduced during this time, so you'll rest in a comfortable, reclined position throughout.

Once the effects fade, we use the rest of the session to process what came up. The experience can be hard to put into words, but the insights tend to feel clear. Studies show the benefits to mood and neural growth can last up to two weeks.

  • Initial consultation. We review your medical and psychiatric history, walk you through the treatment, and determine whether KAP is right for you.

  • Your plan. If you're eligible, we build a personalized ketamine prescription and outcome-monitoring plan. Journey Clinical sends a small supply of oral ketamine to your home — enough for your first two sessions — and their medical team teaches you to take your vitals and self-administer the lozenges before we meet.

  • Preparation. Once your lozenges arrive, we schedule your preparation, dosing, and integration sessions. Prep works like a regular therapy session: we align on the process and set your intentions for the work ahead.

  • Dosing session. A dosing session runs 2-3 hours, in person at our office or remotely by telehealth. You self-administer your lozenge, then settle into a comfortable, reclined position with an eye mask and calming music. The experience is mostly internal — but your therapist stays with you the entire time to hold space and offer support.

  • Integration. After dosing, we meet for several integration sessions to explore the memories, thoughts, and insights that surfaced, and to prepare for your next dosing session.

  • Follow-up. Journey Clinical's medical team schedules regular follow-ups to monitor your outcomes and refill your prescription as appropriate. How often depends on your plan — at minimum, once per quarter.
